Hospital Medicine

We have two primary goals for our Hospital Medicine service: 1) provide exceptional care to our patients and 2) be providing exceptional service to our hospital partners. The Charter Team is composed of several physicians and nurse practitioners with years of experience working as hospitalists at acute care hospitals in the Boston area. In fact, our hospitalist team averages more than 10 years of experience in managing acute care hospitalized patients.

Managing patients with acute illness is both highly rewarding and challenging. Exceptional service requires more than getting the diagnosis and treatment plan right. We must also work effectively with the hospital’s multidisciplinary staff, coordinate care across departments and consultants, and transition patients safely to home with clear discharge plans and follow up with their outpatient providers.
