Comprehensive Geriatric and Rehabilitation Medicine Services
At Charter Health, our Geriatrics team collaborates with skilled nursing facilities to deliver exceptional rehabilitation and geriatric care. Our dedicated team of nurse practitioners and physicians provides medical coverage five days a week, Monday through Friday, ensuring consistent care for our patients.
Who We Serve
Our patients include:
- Long-Term Care Residents: Individuals residing in nursing homes who require ongoing medical care.
- Short-Term Rehabilitation Patients: Those recovering from serious medical or surgical events, transitioning from hospital to rehabilitation to home.
Safe Transitions of Care: Hospital to Rehab to Home
Upon a patient’s admission to rehab, our team performs a thorough review of:
- Hospital course
- Past medical history
- Discharge orders
We then develop a detailed medical care plan and visit patients an average of three times per week. Care plans and recommendations are documented in EPIC, our electronic medical record system, ensuring clear communication with primary care physicians and the care team.
